CagriSema: The GLP-1 + Amylin Combination Coming in 2027
What Is CagriSema?
CagriSema is a fixed-dose combination of two drugs in one weekly injection:
- Semaglutide (2.4mg) — the same GLP-1 agonist in Wegovy
- Cagrilintide (2.4mg) — a long-acting amylin analog
By combining two different appetite-suppressing mechanisms, CagriSema produces more weight loss than semaglutide alone.
How Amylin Differs from GLP-1
GLP-1 and amylin both reduce appetite, but through different brain pathways:
- GLP-1 works primarily in the hypothalamus, reducing hunger signals
- Amylin works in the area postrema and brainstem, promoting fullness after meals
Combining both creates a "two-punch" effect on appetite that neither achieves alone.
Clinical Results
The REDEFINE trials showed:
- CagriSema: 22.7% body weight loss at 68 weeks
- Semaglutide alone: 15.8% at same timepoint
- Cagrilintide alone: 8.6% at same timepoint
The combination is clearly more than the sum of its parts — it exceeds retatrutide's Phase 2 data (24.2%) at similar timepoints, though direct comparison is difficult.
Dosing & Titration
Expected titration (based on trials):
- Weeks 1-4: CagriSema 0.06/0.25mg
- Weeks 5-8: 0.12/0.5mg
- Weeks 9-12: 0.24/1.0mg
- Weeks 13-16: 0.6/1.7mg
- Week 17+: 2.4/2.4mg (maintenance)
Single weekly subcutaneous injection, similar to current Wegovy.
Side Effects
Similar to semaglutide but slightly higher incidence:
- Nausea: ~30% (vs 25% with semaglutide alone)
- Injection site reactions: higher than semaglutide alone
- GI effects generally manageable with titration
When Available?
- Novo Nordisk (same company as Ozempic/Wegovy) is developing it
- FDA submission expected 2026-2027
